By law, all patients are required to provide us their personal details and medical history by filling out the Patient Intake Form below.  All details you providedd will be kept as confidential.  Please bring it with you to your first appointment.

In our practice, Dry Needling is a form of technique that's commonly used as part of the treatment program and to be used only when the patients are able to accept at ease with the use of the needles.  Therefore, prior to your first Dry Needling treatment with us, a Consent Form must be signed by you which can also be downloaded below.

We recommend printing and completing the below forms in advance, but if you are unable to, you may get them when you check in and fill them out in the waiting room.
